EXAMINATION APPOINTMENT REQUEST

   This page is to request an appointment for your pet to be seen by a veterinarian for an examination. This can be for any non-emergency purpose, such as wellness examinations, vaccinations, skin disorders, stomach disorders, and even behavior problems. To ensure you get the appointment you prefer, we recommend you ask for a day that is at least five days from now.
    If this will be your first visit with us, please download a client registration form and fill it out  for you and all of your animal companions. Bring it with you to your first appointment. Arrive about 15 minutes before your appointment is scheduled. If you would rather fill it out in our office, please arrive 20 minutes early.
    Appointments are not confirmed until we have contacted you by phone or e-mail. Thank you!

Please complete all boxes. For existing clients, please enter the name on the acccount for "Your First and Last Name." If possible, indicate a range of dates and times that would work for you. To describe what the appointment is for, type "Annual Exam", "Examine Eyes", Examine Skin", or any other body system that needs help. Please also indicate any other services or medications you will be getting.
