  • Does your pet require medication(s) to be administered during your his or her stay? If so, please outline below the name, strength, and dose of the medication, as well as the frequency of administration. All medication must be in the original bottle. There is a $5.00 charge per day to medicate boarding patients if medications are brought from home and provided by the owner. If medication is not provided, additional charges will apply.
  • If so, please outline feeding instructions. Please also make sure your name is written on the bag or container.
  • Do you have multiple pets that you wish to board or play together?
    By checking yes below, I request that my pets are boarded in the same kennel or run. I realize that when confined some pets can become aggressive, resulting in injuries. I authorize Ebenezer Animal Hospital to treat any wounds that arise from said confinement. I agree to hold Ebenezer Animal Hospital harmless and I assume full responsibility for costs incurred.



  • Would you like your pet bathed on the day of pick up (for an additional charge)? Pets may acquire odors during boarding due to being around other animals. Pets groomed on the day of departure will not be charged for late pickup.

  • All our visitors are VIPs (Very Important Pets), but sometimes they need some extra pampering when they are away from home.

  • For the health of our patients, we require that your pet be up to date on a veterinary strength flea treatment prior to boarding at our facility at the discretion of our team. Proof of appropriate prevention must be provided upon arrival for all cats adn dogs. If proof of prevention can not be provided, the pet will be treated at the owner’s expense ($5-$12) using Capstar (Nitenpyram), a fast-acting oral flea treatment for dogs and cats that kills adult fleas for 24 hours.

  • I grant to Ebenezer Animal Hospital,
    its representatives and employees, the right to take
    photographs of me and/or my pet, and to use and publish the same in print and/or electronically.
    I agree that Ebenezer Animal Hospital
    may use such photographs of my pet and/or me with
    or without my name and for any lawful purpose,
    including, for example, such purposes as publicity,
    illustration, social media, advertising, and Web content.



  • By checking below, I am the owner or agent of the owner and am fully competent to sign this agreement. I authorize Ebenezer Animal Hospital (EAH) to board my pet(s). I understand that my pet must be up to date on all necessary vaccinations and examinations. If verification of current vaccination and examination are not possible, I understand that my pet(s) will receive the required vaccinations and examinations as listed below. I understand that my pet must be parasite free and that any pet found to have a parasite infestation will be treated with a veterinarian approved treatment at my expense.

    All canines must be current on:
    – Full physical exam (yearly),
    – Distemper/Parvo,
    – Bordatella,
    – Rabies
    – Canine Influenza
    – and have a negative fecal test (yearly).

    All felines must be current on:
    – Full physical exam (yearly)
    – Distemper/Resp
    – Rabies
    – and a negative FIV/FELV test

    By checking below, I realize that if I should fail to notify EAH of a delay in my return and fail to retrieve my animal(s) at the agreed time, my pet(s) may be transferred to an animal shelter or other organization for adoption 10 days after notification of abandonment by EAH pursuant to SC Code of Laws §40-60-280. I realize that I will still be responsible for all boarding and collection fees incurred because of such abandonment.
    I understand that EAH does not accept personal items while boarding (toys, blankets, beds, etc) and is not responsible for any personal items left at hospital.
    I authorize EAH to use all resasonable precautions against injury, escape, or death of my pet. The hospital and staff will not be held liable for any problems that develop while boarding provided that reasonable care and precautions are followed. I understand that my pet’s environment will be different from that at home and therefore problems such as barking, abnormal appetite, weight loss or gain, sore throat, diarrhea, or other anxiety related problems can develop.

    MEDICAL ILLNESS POLICY: One of the advantages to boarding your pet at EAH is that veterinary attention is readily available should the need arise. If your pet becomes ill or is suffering from an anxiety related problem, we will call the emergency number(s) listed regarding your pet’s condition, treatment options, and estimate of additional costs. If the EAH staff is unable to reach a responsible party, we at EAH will perform whatever services the doctor deems necessary for the best care for the pet and the expenses involved will be the full responsibility of the pet owner.
    I agree to all terms stated above. I agree to pay for all services rendered upon my return.
